The Optical Assistance Program is offered through Davis Vision/Vision Works. When you see a participating Davis Vision provider you are eligible for the following:

· Eye Examinations every 12 months

· Eyeglasses or Contact lenses every 24 months.

· Every 12 months for dependents up to age 19

What do you get for reimbursement?

The Child Care/Elder Care provides reimbursement up to maximum of $750.00 for the cost of caring for eligible dependents (children and dependent parents) so that you, and if married, your spouse can work.:

What is the death benefit?

There is a $2,000.00 death benefit available for each active employee, spouse and dependent child eligible for Fund benefits. This benefit will be paid directly to the estate of the deceased, provided a death certificate is submitted.
